Local anesthesia is a mainstay in the pain management arsenal in dental offices. Like all other medications, local anesthesia can also result in serious complications, whether they be due to drug toxicity, nerve and vascular injury, unintended nerve involvement or any attendant medical emergency, which may include syncope, psychogenic reactions or myocardial challenge, among others. This clinically relevant presentation will examine all possible complications associated with the use of local anesthetics, including inability to anesthetize.

Course Description: This presentation will introduce the art of administering a propofol-remifentanil admixture by microbolus and continuous infusion to achieve a dose-dependent choice of moderate sedation, deep sedation or general anesthesia. Doses, caveats, limitations, complications and monitoring during this exciting technique will be reviewed. The premise is obvious: when used properly, this technique markedly improves anesthetic control for both short and long cases, thereby increasing safety.

Course Objectives:

  • Describe the clinical actions of remifentanil.
  • Summarize the dose requirements and administration technique for a remifentanil propofol admixture.
  • Explain and discuss at least 5 potential complications and their management when using the propofol-remifentanil anesthetic technique.
